True North Metabolic is a physician-led men’s health clinic. We support patients across Kitchener, Waterloo, Cambridge, Guelph, and surrounding communities, including the Toronto and GTA corridor and London, Ontario. Our approach to testosterone replacement therapy (TRT) is careful, evidence-informed, and personalized. We focus on clear assessment, shared decision-making, and close follow-up.
Our Philosophy
Low testosterone is a medical diagnosis that requires both symptoms and lab testing. We do not prescribe testosterone to men with normal levels. When low testosterone is confirmed, we discuss options that fit your goals and daily routine. Some patients prefer to explore non-testosterone therapies, while others want to consider TRT. Many benefit from a structured plan that includes sleep, nutrition, activity, and stress strategies alongside any medication choice. We recognize that not everyone is the same.
Your First Visit
The first appointment is organized around understanding your day-to-day experience. We review various factors, including:
Energy levels
Mood
Sleep quality
Libido
Morning erections
Exercise tolerance
Weight changes
Medications
We also ask about shift work, alcohol consumption, snoring, possible sleep apnea, and past illnesses. A focused physical exam and a review of prior lab results help us understand the whole picture. If testing is needed, we order morning testosterone on two separate days along with related labs as indicated. The aim is to determine whether low testosterone is present and whether other health factors may be contributing to symptoms.
If your primary care doctor has already confirmed the diagnosis, we can move on to the next steps simultaneously.
When TRT Is Considered
If you have persistent symptoms and testing confirms low testosterone, we talk through the role of TRT in plain language. The goal is to restore physiological levels, not to chase supraphysiological numbers. We review potential benefits, possible side effects, and monitoring requirements so you can make an informed choice. If your testosterone levels are normal, we do not start TRT. Instead, we look for other causes of symptoms and discuss non-testosterone strategies.
Choosing a Treatment Path
There is no single best option for every man. Together, we decide what fits your goals, preferences, and medical context.
Injectable Testosterone: Many patients choose a self-injected option because it is reliable and cost-effective. We teach safe technique, discuss dosing schedules, and plan follow-up to avoid large peaks and troughs. This method is generally the most suitable for many patients.
Alternative Options: Some men prefer to avoid needles. We discuss Androgel or Natesto, for example, when suitable. These options may fit men who want daily, flexible dosing with attention to application instructions and skin precautions. This is ideal for those who explicitly want to avoid injections.
Other Therapies: For selected patients, therapies such as clomiphene or human chorionic gonadotropin may be considered. These can be appropriate in specific situations, such as men who want to explore options that may support endogenous production. Suitability is assessed on a case-by-case basis.
All options include a review of practical factors such as lifestyle fit, cost and coverage, handling, and monitoring. Preferences change over time, so we keep the door open to adjust if life or goals shift.
Risks, Benefits, and Informed Consent
Before any therapy begins, we provide clear information about potential benefits and risks. Topics typically include:
Red blood cell changes
Skin reactions with gels
Changes in fertility parameters
Other considerations such as sleep apnea or prostate monitoring based on age and risk
We invite questions and encourage patients to take their time to decide. Our goal is to support an informed, comfortable choice.
Follow-Up and Monitoring
Monitoring keeps therapy safe and effective. We schedule labs at set intervals and adjust the plan only when results are back. Follow-ups can often be virtual for many patients in Kitchener, Waterloo, Cambridge, Guelph, the Toronto and GTA area, and London, Ontario, once a baseline is established. We do require in-person follow-ups for many cases but take geographic location into consideration. We review symptom changes, energy, sleep, training tolerance, and practical concerns such as work travel or shift schedules. If adjustments are needed, we explain the rationale and confirm a new monitoring timeline.
A Visit Flow at a Glance
Symptoms and History: We document what brought you in and what you want to improve.

Consent and Start: If you choose to proceed, we set a plan that includes dosing, education, and a monitoring schedule.
Follow-Up: We review labs, adjust dosing when indicated, and address questions. Virtual visits are available when appropriate.
Coordination with Your Primary Care Team
We look to collaborate with family physicians and specialists across Kitchener–Waterloo, Cambridge, Guelph, the GTA, and London. With your permission, we share updates so that medication lists and monitoring remain aligned. If another condition needs attention, we will recommend follow-up with your existing clinicians.
